Creating A New Registry

Creating A New Registry for that special occasion is very simple at Pfaltzgraff-Dinnerware.  Just complete this form and send it to us.  We will then create a Registry just for you, and then contact you to have you review and approve it.  Once that's done, all you have to do is e-mail your friends and tell them to look up your Registry at Pfaltzgraff-Dinnerware.  It's that simple!

(1) Your Name (if Wedding, please enter both bride and groom names): 

(2) Occasion: 
(3) E-Mail Address: This is the address where we should write to you so 
you can review the Registry after it has been created.
 
Preferred Ship to Address as it will appear on the registry:
(4) Street: 
(5) City: 
(6) State:  (7) Zip:(+4 if known) 
(8) Dinnerware Pattern Selected.  Please be as specific as possible, but don't 
worry; once your Registry is created you will have the opportunity to edit 
it for correctness.  Please list specific quantities of each item you 
want, or tell us how many table settings you would like and we will add in 
the correct number of place setting pieces.


(9) After clicking the "Submit" button, a screen will appear to inform you 
if your submission was successful.  You can then use the "Back" button on 
your broswer to return to this screen.
(10)Click the SUBMIT button below to send your Registry submission.
Once your form is submitted, we will contact you within 48 hours and let you know when you can review your Registry.
